Assessment Coordinator I

Under the administrative supervision of the Program Director or designee, this position is responsible for completing initial and annual assessments, providing psychosocial, rehabilitation, and recovery-oriented services for adults.

Essential duties and responsibilities include:

• Assisting members in identifying, expressing, asserting, and communicating their goals and needs while living in the community
• Completing the intake and renewal Core Assessment, including collateral assessments in the field or in the office in coordination with the client's multi-disciplinary team
• Facilitating the development of the Individual Client Plan in collaboration with the member and the treatment team
• Providing ongoing consultation to the treatment team and other staff to support the delivery of services that are driven by the member's client plan and supportive of the member's goals
• Completing all required paperwork, including annual member paperwork as assigned, promptly
• Driving on program business using personal vehicle for outreach and engagement, assessments, medication outreach, transporting clients to appointments/activities, community/county meetings/trainings
• Providing prompt intervention in the event of a crisis, both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis
• Participating in off-site trainings and providing in-service training for all staff
• Conducting Quality Management activities as directed by the Clinical Director and Team Leader
• Facilitating groups, providing individual and family therapy, and updating diagnoses for clients not receiving medication management services from the program psychiatrist, with oversight from a licensed professional if not themselves licensed
• Attending to the safety, health, and well-being of clients
• Completing paperwork as assigned on time, and maintaining productivity standards outlined for the program
• Providing after-hours, "on-call" coverage on a rotational basis to program clients

Minimum qualifications and certifications and licenses:

Education, training, and experience: A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from a four-year college or university with a bachelor's degree in psychology, Social Work or related field, and a minimum of four years' experience as a provider of mental health services; preferably working with all demographics, populations, and ages (TAY, adults, and older adults). Experience in conducting functional assessments, bio psychosocial assessment, and other clinical approaches in interviewing and gathering data based on mental health needs, diagnosis, and functional impairments. Basic computer literacy is essential.

Licenses; certificates; special requirements: California driver's license & current vehicle insurance/registration Reliable means of transportation capable of passing vehicle safety inspection if more than five years old, excluding all modes of two-wheeled transport, inclusive of bicycles, mopeds, and motorcycles. Bachelor's Degree and 4 years of relevant experience or, Master's Degree and 2 years of relevant experience.

Schedule: Tuesday through Saturday from 8:30 AM - 5:00 PM. Compensation: $24.00 - $24.97 an hour.
