Wellness Director (LPN/RN)

We're hiring a Wellness Director for a family-owned assisted living and memory care community-where you'll lead a stable team, build real relationships with residents and families, and operate in an environment that values quality care over chaos.

What This Role Looks Like

• Lead daily clinical operations in a 53-bed assisted living and memory care community
• Supervise and support caregivers, med techs, and clinical staff
• Oversee medication management and ensure compliance with SC DHEC regulations
• Conduct resident assessments and update Individual Service Plans (ISPs)
• Coordinate with families, physicians, and the Executive Director
• Drive quality assurance and ensure consistent standards of care
• Manage staffing schedules and ensure appropriate coverage

What You Need:

• Active LPN or RN license (South Carolina required)
• 3+ years in assisted living, memory care, or long-term care
• 2+ years in a leadership or supervisory role
• Strong understanding of DHEC regulations
• Experience with care planning, assessments, and medication management

Additional Details:

• Must be comfortable with occasional evenings/weekends as needed
• Background check and drug screening required
