Hospice Case Manager (RN)

We're hiring a Hospice Case Manager (RN) who wants a predictable weekday schedule, meaningful patient relationships, and the autonomy to provide compassionate care in patients' homes.

You'll step into a field-based hospice role providing case management throughout Gwinnett County. You'll conduct assessments, develop care plans, manage symptoms, educate patients and families, and coordinate care while working alongside a supportive interdisciplinary hospice team.

Hospice experience preferred, with training available for strong RNs from Home Health, ICU, CCU, Oncology, or Med-Surg backgrounds

You hold an active Georgia RN license

You maintain current CPR certification

You have at least 1 year of nursing experience

You're comfortable providing independent, compassionate care in patients' homes

You have a valid driver's license and reliable, insured transportation

You value patient-centered end-of-life care and supporting patients and families

Compensation & Benefits

Base salary: $90,000$95,000

Mileage reimbursement

Cell phone stipend: $60 per month

Health insurance: Employer pays 75%

Dental insurance: $7.48 per pay period

Vision insurance: $1.37 per pay period

Paid time off: 17 days in Year 1, increasing to 23 days beginning in Year 2
