As an Assessment Specialist Flex, youll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Youll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community./ppKey Responsibilities/pulliCompletes visits to assess for eligibility for specified program and conducts safety assessments, in compliance with CareStar and department guidelines./liliDocuments all applicant and case-related contacts and interventions in applicants record per agency and CareStar policy, including standards of content and timeliness./liliProcesses specific Program Eligibility Assessment Tool (PEAT) or other tools and all related documents per agency and CareStar policy./liliProvides assistance with community resource referrals as appropriate./liliIdentifies and reports high-risk situations as required per agency and CareStar policy./liliAdhere to the CareStar Rule in performance of job responsibilities./liliUnderstand and comply with CareStar Policies and Procedures./liliMaintain confidentiality as related to patient information.
