Candidates must have two years of experience in a home setting within the last fifteen years. Nurses should have recent experience performing comprehensive assessments of patients. The role involves reviewing health status questions and making evaluations and determinations based on UAS criteria for enrollment. Candidates should demonstrate the ability to work with technology in the workplace. Knowledge of the theory, principles, and practices of general professional nursing is essential. Familiarity with state and agency laws and regulations governing nursing practices is required. Excellent organizational, interpersonal, written, and verbal communication skills are necessary. Candidates must be able to communicate and work effectively with multi-generational consumers. Demonstrating sensitivity to the concerns of others is essential. The ability to perform comfortably in a fast-paced, deadline-oriented work environment is important. Applicants must be capable of executing complex tasks simultaneously and working independently. Responsibilities include performing initial in-home pre-evaluations of patients. The evaluations will assess health status, strengths, care needs, and preferences. The evaluations will guide the development of individualized long-term care service plans. Reviewing medical documentation and health referral forms relevant to the case is necessary. Evaluation data must be entered into an electronic evaluation form and transmitted as required. Concerns, conflicting information, and issues that arise during the evaluation process must be documented. Emphasizing continuity of care is critical to reduce fragmentation, duplication, and gaps in treatment plans. Discussions with consumers about healthcare options and supports needed will be part of the role. Communication between the Call Center and eligible consumers for enrollment in a managed care health plan must be initiated. Enhancing communication and collaborative relationships with interdisciplinary care team members is essential to improve care coordination. Quality review data and required documentation to support outcome measurements must be collected. Maintaining a comprehensive working knowledge of community resources and services for the target population is necessary. Meeting all performance criteria established for the position is expected. Additional duties may be assigned by the Regional Nurse Manager or project management as needed.
