CASE MANAGER

Sevita is seeking a compassionate Case Manager to support individuals in our community-based healthcare programs. In this role, you will conduct person-centered assessments, develop individualized service plans, and coordinate medical, behavioral, and social supports to help people reach their goals. You will connect individuals and families with community resources, monitor progress, and advocate for needed services. Working in a collaborative, inclusive environment, you’ll partner with interdisciplinary teams, maintain accurate documentation, and participate in ongoing training to grow your skills and impact.
