We offer patients convenient access to innovative treatments and advancements that improve the health and overall well-being of our community.ResponsibilitiesPerform focused patient assessments within scope of practiceMonitor and document vital signs, pain, intake/output, and clinical changesProvide basic nursing care (bathing, toileting, repositioning, ambulation)Assist with activities of daily living (ADL's)Reinforce patient comfort and safety measures (fall precautions, skin integrity)Administer oral, topical, subcutaneous, and intramuscular medications within scope of practiceMay administer IV medications/fluids after completion of mandatory IV class offered during orientationPerform wound care (dressing changes, sterile technique)Carry out ordered treatments (catheter care, suctioning, tube feedings)Observe for changes in patient conditionReport abnormal findings promptly to the RN and or providerDocument accurately in the EMRParticipate in early recognition of deterioration (sepsis, stroke symptoms)Actively participate in Code Blue and Rapid Responses on unit as neededCollaborate with RNs, CNAs, physicians, therapists, and case managersCommunicate effectively with patients, family, RNs, LPNs, providers, ancillary, CNAs and case managementCollaborate with RNs, CNAs, physicians, therapists, and case managers to meet patient care needsImplement components of the nursing care plan developed by the RNDelegating task to CNA's when appropriateParticipate in shift reports, bedside handoff, and huddlesReinforce education provided by the RN or providerInform patient on medications being administered and related side effectsMaintain timely, accurate, and complete documentationEnsure all care aligns with the State Nurse Practice Act, Facility policies, and Regulatory standards (Joint Commission)Understand that documentation is a legal record of care and maintains HIPPA and patient confidentialityAdhere to standard and transmission-based precautions per infection control policiesPerform hand hygiene and PPE useMonitor for and help prevent hospital acquired conditionsSupport patient safety goals aligned with quality measuresAssist with room preparation for new admissions and keeping patient rooms tidy throughout shiftAssist with specimen collections as ordered (urine, stool, sputum, and would cultures)Contribute to workflow efficiency in high-acuity environmentsAssist with postmortem care when requiredAssist with patient transport (discharge area, etc.)Support organizations quality outcomes and patient experience measuresCompletes all mandatory department, educational and hospital requirementsAdheres to current Infection Control and Safety StandardsRegular and prompt attendanceAbility to work schedule as defined and overtime as requiredRelated duties as assignedQualificationsGraduation from an accredited school of practical nursing meeting the minimum standards set by the State of Alabama RequiredLPN - Licensed Practical Nurse - State Licensure - Current Alabama Licensed Practical Nurse (LPN) license or current Multi-State LPN License in accordance with the Nurse Licensure Compact (NLC) for Alabama Upon Hire Required andBasic Life Support (BLS) issued through on organization that requires a hands-on instructor lead psychomotor skills verification (ex.
