Physical Therapist - Travel Contract

Minimum Requirements Bachelor's, Master’s or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Indiana State Physical Therapy license is required to start the assignment. BLS Certification from AHA or ARC may be required. Outpatient physical therapy experience preferred, including evaluation, plan of care development, and functional treatment planning. Experience with documentation of evaluations, goal writing, letters of medical necessity, and charge entry in compliance with payer and clinic policies preferred. Ability to supervise and delegate to support staff and to participate in student clinical education preferred. Maintain professional licensure and complete state-mandated continuing education. Adherence to state Practice Act and professional code of conduct required.
