Job Summary:

Viventium Home Care Agency is seeking a compassionate and skilled Registered Nurse to join our team in New York, New York. As a dedicated home care provider, we pride ourselves on delivering high-quality patient care and support to those in need. Responsibilities:

• Provide direct patient care to individuals in their homes, focusing on promoting health, preventing illness, and alleviating suffering

• Conduct initial and ongoing assessments of patients' physical, emotional, and social needs

• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams

• Administer medications, treatments, and therapies as prescribed by physicians and medical directors

• Collaborate with interdisciplinary teams to ensure comprehensive care and continuity of services

• Maintain accurate and timely documentation of patient care and services rendered

• Educate patients and families on disease management, self-care, and health promotion strategies

• Participate in quality improvement initiatives to enhance patient outcomes and agency performance

Requirements:

• Current New York State Registered Nurse license

• BLS (Basic Life Support) certification

• Ability to lift up to 50 pounds and perform physical tasks as required

• Excellent communication and interpersonal skills
