Participant Onboarding & Care Advocate

Habitat Health seeks a New Participant Advocate to welcome and onboard participants into the PACE program. You will build trusted relationships with participants and caregivers, coordinating with the Interdisciplinary Team to ensure a smooth transition into care and ongoing support. You will educate clients about services, coordinate appointments and transportation, and document interactions in the EMR.
