About the Role:

• As an LVN, you will work on a per diem basis, providing direct patient care and supporting our nursing team in delivering high-quality care to our patients.

• You will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care transitions.

• Your expertise and compassion will make a significant difference in the lives of our patients and their families.

Responsibilities:

• Provide direct patient care, including administering medications, performing treatments, and monitoring patient condition.

• Collaborate with other healthcare professionals to develop and implement individualized care plans.

• Conduct patient assessments, monitor vital signs, and report any changes or concerns to the nursing team.

• Participate in quality improvement initiatives to ensure excellent patient outcomes.

• Maintain accurate and detailed records of patient care and progress.
