They are happy to find ways to maximize the patient’s outcome, be a patient advocate, and communicate as needed with the primary doctor, our Clinical Team Director, Director of Nursing and other team members to manage the patient’s care. What You’ll Do Deliver personalized, in-home therapy to enhance mobility and quality of life for seniors. Collaborate closely with patients, families, and our care team to ensure seamless, timely treatment plans. Maintain clear, consistent communication to keep everyone informed and empowered. Strong organizational skills and a commitment to being on time, every time. Excellent verbal and written communication skills to connect with patients and staff.
