Details Home health setting covering Van Buren, Calhoun, and Kalamazoo counties Full-time day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM Weekend rotation required every 4-6 weeks No call requirement 13-week contract assignment Requirements Active Michigan PT license required (license required if applicable) Current BLS certification (AHA) required Minimum 1 year of home health experience required Valid, active driver's license in good standing required Daily access to a personal vehicle for business use and proof of auto insurance required Ability to evaluate, plan, and administer PT treatment for patients with injury or musculoskeletal, neurological, and orthopedic conditions Additional Information Responsibilities include evaluating patient function, establishing treatment plans, and documenting care in the electronic health record per compliance timelines May design and fabricate splints or other assistive devices as needed Provides family and community education and participates in interdisciplinary team discussions May cross-train and provide coverage at other departments within Rehabilitation Services as needed Serves as a resource and provides in-services in areas of clinical expertise Proof of flu vaccination required with consideration; exemption requests must be submitted within 48 hours of signing References must be from a supervisor-level contact RTO limited to no more than 10 days Local candidates accepted at the same rate with no radius restrictions Previous internal travelers may return after 6 months; former permanent staff may return after 1 year American Traveler Job ID P-747984.
