Description Responsibilities Perform in-home assessments and ongoing health monitoring Deliver skilled nursing procedures: wound care, IV therapy, catheter care, injections, G/J tube care, tracheostomy and ventilator management Document care using Alora (EHR system) and follow established care plans Collaborate with families, case managers, and providers for coordinated care Offer patient/family education on self-care, medication management, and recovery What We Offer Flexible scheduling (8–12 hour shifts, day and night options) Immediate cases available in metro and rural counties Supportive team and responsive administrative staff Opportunities for continuous training 24/7 on‑call nurse support for field staff Requirements Active RN license in the state of Minnesota CPR certification (required) Recent TB test or screening Prior home care or private duty nursing experience (preferred)Reliable transportation and willingness to travel to client homes Strong communication and documentation skills #J-18808-Ljbffr BetterHealth LLC
