Registered Nurse - Hospice

Job Description

Job Summary:
Provides specialized nursing care to hospice patients, assessing their needs, developing and implementing care plans, and ensuring quality end-of-life care in accordance with regulations and agency policies.

Responsibilities:

• Conducts comprehensive patient assessments to determine eligibility and identify care needs.
• Develops, implements, and regularly updates individualized care plans in collaboration with interdisciplinary team members.
• Ensures appropriate utilization of hospice services and maintains documentation of patient progress.
• Performs patient education tailored to age and cognitive level, documenting teaching strategies used.
• Manages case activities including prioritizing patient needs, medication reconciliation, and ensuring compliance with pharmacy regulations.
• Participates in team meetings, quality improvement activities, and provides mentorship to new staff.

Qualifications:

• Must have a valid New Jersey RN license with at least 2 years of experience as an RN in a home hospice environment.
• Demonstrates proficiency in using EMR systems with excellent critical thinking and communication skills.
• Holds a valid New Jersey driver's license.
• Holds BLS certification.
• Knowledge of hospice regulations, insurance reimbursement, and pharmacology required.
