We are seeking a compassionate and experienced Medical Provider with Family Medicine or Internal Medicine specialty to join our mission-driven team at a Federally Qualified Health Center in Wailuku on the Island of Maui. This full-time exempt role focuses on adult medicine in a primary care setting serving a diverse community with a strong emphasis on prevention chronic disease management and whole-person care.

The FNP/APRN/MD/DO is a vital member of the clinical team and works with the providers and staff to deliver patient care in accordance with nationally accepted guidelines policies and procedures to achieve the health centers goals and objectives in delivering exceptional care to our patients.

The salary range for MD/DO is $220000 to $240000 year plus a Sign-On Bonus of $15000 and eligible for a fiscal year incentive bonus up to $25000.

We provide Adult Medicine Primary Care as a federally qualified health center (FQHC) on Maui.

The FNP APRN MD or DO provider is a vital member of the Adult Medicine clinical team to deliver patient care in accordance with nationally accepted guidelines policies and procedures to achieve the health centers goals and objectives in delivering exceptional care to our patients.

• Performs physical assessments of patients with acute and chronic diseases and illness; provides a treatment plan and follow-up with written protocols.
• Educates and guides patients on disease prevention and healthy lifestyle habits.
• Performs screening history and physical examinations for patients including those who are homeless and aging.
• Performs diagnostic tests and screening evaluations.
• Provides or makes arrangements for the provision of pharmaceuticals based on written protocols or direct physician order.
• Provides referrals to appropriate sources for specialized or chronic medical care.
• Investigates and intervenes into potential and actual problem situations regarding patient care.
• Communicates conferences and collaborates patient care with and among patient patients family physicians and other health care team professionals.
• Documents nursing care according to established guidelines and maintains patient medical records (EMR) and charts.
• Participates in Quality Improvement program in-service meetings committees and research activities as required and assigned.
• Performs fiscal duties submits reports for funding and grant proposals writes reviews and revises procedures relating to patient care and other assigned administrative duties as required to support primary care and in collaboration with the related integrated health services offered by the clinic.
• Current CPR negative TB within past 12 months and APRN or NP or MD or DO licensure/NED registration in the State of Hawaii required.

The ideal candidate will have APRN/FNP/DNP/MD/DO from a recognized program evidence of continuing education competencies with providing services to a diverse patient population as a provider for at least one year and verifiable work background in treating all individuals with respect and aloha. This position reports to the Department Head for Adult Medicine at our Wailuku clinic. We currently serve adult patients at three locations on Maui. This is a Full-time Exempt position with a rotation of after hour call duty and half-day Saturday shift every 6 to 7 weeks and one late shift per week on the same day each week (late shift is 10 AM to 7 PM) shifts are 8 hours and seeing at least 16 patients per shift on average.
