Responsibilities

• Provide skilled nursing care to patients in their homes according to individualized care plans
• Perform comprehensive patient assessments and monitor changes in health status
• Administer medications, IV therapies, wound care, and other prescribed treatments
• Develop, implement, and update individualized care plans based on patient needs
• Educate patients and caregivers on disease management, medications, treatments, and home care techniques
• Coordinate care with physicians, therapists, case managers, and other members of the interdisciplinary healthcare team
• Maintain accurate and timely documentation using electronic medical record systems
• Monitor patient progress and communicate significant changes to the healthcare team
• Ensure compliance with home health regulations, infection prevention standards, and agency policies
• Promote patient independence, safety, and overall well-being during home visits

Requirements

• Active RN License in California
• Minimum 2 years of recent Home Health nursing experience
• Current BLS Certification required
• Valid driver's license, reliable transportation, and auto insurance as required for home visits
• Experience with electronic documentation systems preferred
• Strong assessment, clinical judgment, and critical thinking skills
• Excellent communication and patient education abilities
• Ability to work independently and effectively manage a daily caseload
• Strong organizational and time management skills
• Commitment to providing compassionate, patient-centered care
