No Clinical Involvement - Profiles sent directly to the manager for phone interview MUST HAVE HOME CARE HOME BASE EXPERIENCE AND HOME HEALTH EXPERIENCE Department: Physical Therapy How many points a day/week? Do you require travelers to do "start of care", recerts, daily visits? 6.5 Day; Yes - SOC, ROC, Recert, Evals, visits What certifications do you require? BLS Common diagnosis / Types of patients CHF, ORTHO, PNEUMONIA What areas are they required to cover? Home Health Experience or 'Must have' skills & common job duties: No requirement but would prefer no new grads Is there a resource available for them to reach out to while in the field? Yes, Clinical manager or educator 24/7 for emergency or 8-5pm Shifts & Scheduling: 5-8 hour days How often is someone expected to work the weekend shift? What is the weekend shift hours? Yes 8-5 - rotating schedule Are they asked to go into the Holiday Rotation? Yes Scrubs (color?) and/or business casual? Either How many shifts of orientation does someone get with a preceptor? 2 weeks - few visits with a workcoach Additional Notes: Phone and tablet will be provided
