- Position summary:
- The Hospice Social Worker is responsible for providing initial psychosocial assessments, plan of care, ongoing psychosocial counseling, bereavement services, community education and outreach and referral to Hospice patients and families.

- Education:
- Master's Degree in Social Work and is a Licensed Independent Social Worker.
- Required length and type of experience:
- Minimum of three (3) years of clinical work experience preferably in a healthcare setting or other related field.

- Licensed Independent Social Worker.
