RN Case Manager

Job Summary


Our client is seeking a Registered Nurse Case Manager responsible for providing professional, patient-centered nursing care in the ambulatory setting. The role involves working in accordance with the multi-disciplinary plan of care and established policies and procedures under the supervision of the Lead Nurse. This includes direct nursing and telephonic services to patients and families.


Key Responsibilities

• Provide patient care in line with Ambulatory Care Nursing standards.
• Collect and analyze patient data and assist in care plan development.
• Evaluate and document outcomes of care plan interventions.
• Communicate interventions and outcomes per professional and organizational standards.
• Delegate tasks to other licensed and unlicensed care providers appropriately.
• Ensure compliance with regulatory body standards including Joint Commission and PCMH.

Qualifications

• Current Maryland Nursing License.
• Graduated from an accredited Nursing Program.
• Active BLS certification.
• Minimum 1 year of previous ambulatory experience.
• BSN is preferred.
• Experience with EMR systems is preferred.

Schedule

• Monday-Friday 8:00am-5:00pm
• One late night per week: 11:00am-8:00pm
