Job Title Healthcare Consultant I - North Miami area Location North Miami, FL Job Description Assess, plan, implement, and coordinate case management activities with members to evaluate medical needs. Manage a caseload that includes supportive and medically complex members. Develop proactive plans to address issues and enhance member wellness outcomes. Coordinate services including prior authorizations and medication reviews. Conduct visits with members at home, assisted living facilities, and nursing homes. Schedule and Travel Schedule: Monday-Friday, 8:00am-5:00pm. Position requires approximately 75% travel within the region. Fully remote role with no onsite requirements. Qualifications Verifyable High School Diploma or GED required. Must be fluent in English and Spanish (speaking and writing). Benefits Benefits are available to full-time employees after 90 days of employment. A 401(k) with company match is available after 1 year of service. #J-18808-Ljbffr
