About the Position

Job Title: Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 2 years of acute hospital care management experience License: Active Registered Nurse License Certifications: Current certifications as required by the specialty Must-Have: Experience with care coordination and discharge planning Proficiency in computerized documentation Experience with assessing patients Description: Weekend requirement is typically one full weekend every four weeks, subject to departmental needs. Patient age groups include adults and geriatrics. Daily case load may be up to 25 patients. Experience with health maintenance organizations and independent practice associations is preferred. Candidates must demonstrate skills in care coordination and discharge planning. Knowledge of acute hospital admission criteria is essential. Familiarity with CMS guidelines, CPT coding, and HIPAA regulations is required. Experience in managing goals and plans of care, including medical necessity assessments, is critical. Travel may be required between facilities within the designated state. Float among facilities may be necessary as part of the role. Communication devices may be provided to complete assigned duties. Finally, commitment to work on holidays is required as outlined in job commitments.
