Description

• Conduct assessments for patients in either inpatient or outpatient settings and create personalized treatment plans
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Educate patients and families on therapeutic goals, discharge processes, and home exercise protocols
• Maintain meticulous documentation of evaluations, treatments, and progression in line with facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from a recognized program

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurological, geriatric) are an added advantage but not mandatory

Skills

• Prior experience in a relevant setting is strongly preferred
