Your Role as a Home Health Licensed Clinical Social Worker

As a Licensed Clinical Social Worker at Mobile Medical, you will play a vital role in providing holistic care and psychosocial support to patients. Your expertise will help improve health outcomes and address barriers to care through collaboration, counseling, and resource coordination.

Responsibilities include:

• Deliver holistic psychosocial support and behavioral health counseling/therapy to patients.
• Assess and coordinate community resources for patients with socioeconomic needs.
• Develop and implement treatment plans for patients with mental health conditions or other barriers to improved health outcomes.
• Communicate effectively with team members, providers, patients, and the community to ensure continuity of care.
• Provide excellent customer service while collaborating with patients, families, and team members in coordinating care.
• Maintain compliance with medical record standards, policies, and Medicare documentation guidelines.
• Participate in organizational growth initiatives and additional responsibilities aligned with your strengths.

Qualifications

• Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) license
• Minimum two years of clinical counseling experience (required)
• Home health or telehealth experience preferred
• Valid driver's license, reliable transportation, and auto insurance
