- Conduct evaluations for patients in inpatient and outpatient settings and create tailored treatment plans.
- Implement physical therapy interventions that enhance mobility, strength, balance, and overall function.
- Work in unison with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes.
- Guide patients and their families through therapeutic goals, discharge planning, and at-home exercises.
- Maintain detailed documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards.
- Participate in multidisciplinary care team rounds and conferences to ensure seamless care continuity.
- Doctorate in Physical Therapy (DPT) or a degree deemed equivalent from an accredited institution.
- State licensure as a Physical Therapist .
- CPR certification is preferred.
- Specialized certifications such as neurological or geriatric are beneficial but not mandatory.
- Prior experience in a clinical setting is highly desired.
- Capacity to manage patients with intricate medical and mobility challenges.
- Excellent communication and collaborative skills.
- Proficiency with digital charting and clinical documentation technology.
