Description
  • Conduct evaluations for patients in inpatient and outpatient settings and create tailored treatment plans.
  • Implement physical therapy interventions that enhance mobility, strength, balance, and overall function.
  • Work in unison with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes.
  • Guide patients and their families through therapeutic goals, discharge planning, and at-home exercises.
  • Maintain detailed documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards.
  • Participate in multidisciplinary care team rounds and conferences to ensure seamless care continuity.
Education
  • Doctorate in Physical Therapy (DPT) or a degree deemed equivalent from an accredited institution.
Certifications
  • State licensure as a Physical Therapist .
  • CPR certification is preferred.
  • Specialized certifications such as neurological or geriatric are beneficial but not mandatory.
Skills
  • Prior experience in a clinical setting is highly desired.
  • Capacity to manage patients with intricate medical and mobility challenges.
  • Excellent communication and collaborative skills.
