Family Specialist

JOB DEFINITION:

Work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches.

EDUCATIONAL REQUIREMENTS:

A bachelor's degree or higher in social work, psychology, rehabilitation counseling, or related field preferred.

ADDITIONAL REQUIREMENTS:

• Current New York State Driver's License
• Clean DMV Record
• Proof of Motor Vehicle Insurance

EXPERIENCE REQUIRED:

Two years' experience working with the chronically mentally ill (Masters level) recommended. Three years' experience working with the chronically mentally ill (Bachelor's level) recommended. Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.

DUTIES AND RESPONSIBILITIES:

1. Perform routine team duties, and have lead responsibility for integrating dual-recovery treatment, family support or vocational/employment training with the tasks of other team members.
2. Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between FACT and other systems.
3. Engage in performing assessments, case management, handling crisis intervention, and discharge planning.
4. Maintains case records and completing documentation on time and in a matter in compliance with federal, state and agency regulations.
5. Provides direct, Telehealth face-to-face and/or collateral services to recipients either in the FACT Office or in the community in places and situations where problems arise.
6. Will work a 35-hour work week, including responsibility for periodic pager coverage to be arranged in advance with the Team Leader or Assistant Team Leader as needed.
7. Assists recipients in developing appropriate expectations from treatment.
8. Strengthens the ability of the recipient to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination process.
9. Exchanges information with other agencies that benefits the recipient, or ensures compliance with regulations and coordination of services.
10. Utilizes and understand the value of supervision, which increases practice skills, promotes better services for the recipient and preserves accountability to the Agency.
11. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
12. Complete QI Training during onboarding and participate in QI refresher training as needed.
