Care Coordination Case Manager
Disability Supports Of The Great Pl
Disability Supports Of The Great Pl seeks a coordinator to manage communication, services, and support for assigned individuals. You will develop PCSPs, coordinate services, and monitor progress, while participating in clinical and billing meetings as required.
The role requires 2 years in human services, a Bachelor's in a related field, and strong written/verbal skills. Travel to appointments and conferences may be needed.
