Role Description We have openings for full-time, part-time and per diem Home Care Physical, Occupational and Speech therapists. The Home Care Therapist will evaluate patients in their home environments, develop individualized treatment plans. Daily tasks include performing comprehensive assessments, setting measurable goals, documenting visits and progress notes in accordance with regulatory and agency standards, and coordinating care with nursing, home health aides, and other interdisciplinary team members. Qualifications Demonstrated experience in patient care with the ability to deliver safe, compassionate services in diverse home environments. Working knowledge of Medicine as it relates to functional mobility, chronic conditions, and post-acute recovery in home-based settings. Current state licensure (or eligibility to obtain), with good standing and no restrictions. Excellent communication, time management, and organizational skills, with the ability to work independently and as part of an interdisciplinary team. Comfort with electronic health records and digital tools for documentation, scheduling, and remote coordination. Valid driver’s license, reliable transportation for home visits, and ability to travel within assigned service areas. #J-18808-Ljbffr
