On-Call Hours: 6:00 PM – 9:00 AMAbout the RoleWe're looking for a skilled and compassionate LPN/LVN to provide in-home nursing care under the supervision of a Registered Nurse (RN). You'll support patients with daily care needs, monitor their condition, and help ensure safe, high-quality home health services.ResponsibilitiesProvide nursing care according to the RN-developed Plan of Care.Monitor and document changes in patient condition; report to RN/physician as needed.Assist with activities of daily living to promote patient independence.Educate patients and caregivers within LPN/LVN scope of practice.Maintain accurate EMR documentation.Participate in case conferences and coordinate care with the home health team.Follow infection-control, HIPAA, and company policies.Required QualificationsCurrent LPN/LVN license in the state of practice.Graduate of an accredited practical/vocational nursing program.Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.Current CPR/BLS certification.Valid driver's license, reliable insured vehicle.Strong communication, clinical judgment, and ability to work independently.Why Join Us?We are committed to our employees' well-being, offering a comprehensive employee benefits package that includes, but is not limited to:Competitive salary and comprehensive benefits package: EAP and 401(K) with 4% match, accrued sick hours, mileage reimbursementEmployee referral program and opportunities for growth and advancementThis position is based on an hourly visit compensation model.Hourly visit ranges from $33 to $42 visit commensurate with experience and depending on type of visit.
