Schedule: 08:00 - 16:30

Contract Length: 13 weeks

Candidate Type: Traveler

Requirements: Must have: candidates without these skills will not be considered for the role. • Acute Care Case Management Experience (at least 1 year)

Responsibilities:

• Manage patient cases in a hospital setting, ensuring smooth care transitions.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Document patient information accurately using Meditech and Careport EMR systems.
• Provide solutions-focused support to patients and staff for optimal outcomes.
• Adapt to varied case management assignments within the hospital environment.
