Job Description
Physical Therapist (PT) The Physical Therapist will perform comprehensive assessments, differentially diagnose, and identify deficits in patients' mobility and functional activities. The therapist will create individualized treatment plans focused on patient-centered goals aimed at reducing pain, restoring function, and preventing disability while working together with patients to maintain mobility. Essential Functions and Responsibilities:
- Conduct patient evaluations by obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to gather data.
- Develop an individualized plan of care, selecting treatment techniques that address specific impairments identified during evaluations and in line with physician orders.
- Perform ongoing reassessments of patients and monitor responses to treatment.
- Initiate and manage the discharge plan from the first visit onwards, continuing through the course of treatment.
- Supervise and delegate tasks to ancillary staff and students in accordance with state laws and department policies.
- Complete timely, accurate, and clear documentation as per departmental policy and accreditation requirements.
- Communicate effectively with patients, families, caregivers, significant others, and health care team members to maximize the benefits of care, utilizing AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You).
- Assess educational needs of patients and their support networks, providing necessary education to address those needs.
- Engage in professional development by attending educational programs, in-service meetings, workshops, and reading current literature within the scope of practice.
- Actively participate in quality improvement initiatives.
- Ensure a safe environment for patients, visitors, and staff by complying with established safety standards.
- Function within the scope of practice as defined by state practice acts.
- Perform other related duties as assigned.
