Opportunity:

Physical Therapist - Physical Therapy

Job Details:

• Work Monday through Friday, 8:00 AM to 5:00 PM schedule
• 100% remote position within designated coverage area after initial orientation
• 2-day onsite orientation in Oak Brook, IL required
• See 6 patient visits per day within coverage area
• Provide physical therapy services to adult/geriatric general medical/surgical patients
• Work with current census of approximately 200 patients
• No weekend rotation, call, or holiday requirements
• All meetings held virtually with no physical office reporting
• Must provide own reliable transportation to/from all patient visits
• Valid driver's license and active auto insurance required
• 2 years physical therapy home health experience with adult/geriatric patients required
• Active Illinois PT license and BLS (AHA) certification required
• Master's degree in Physical Therapy required
• Must assist with ADLs and adaptive equipment
• Collaborate effectively with other specialties including RN, OT, and Speech
• Work within Central Metro E coverage area: zip codes 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas
• Use EPIC EMR system and Sapphire IV pumps
• Dress code: solid color scrubs acceptable or business casual, no jeans or open toe shoes
• Supplies and equipment delivered to home address
