Montefiore Nyack Hospital in Nyack, NY is seeking an RN Care Manager to collaboratively implement and monitor individualized patient care plans from admission through post-discharge, coordinating services across the continuum of care. Qualifications include a NY RN license and 4–6 years of inpatient med-surge/ICU/ER experience; bachelor or associate degree; strong assessment, communication and time-management skills.
