Seven Healthcare are seeking a dedicated Physical Therapist for a contract assignment in Canonsburg, PA. This role offers an exciting opportunity for a qualified professional to perform comprehensive physical therapy evaluations and treatments, ensuring patients achieve their optimum level of independence and function in a supportive work environment.

- Perform physical therapy evaluations and treatment based on physician orders/referrals.
- Complete comprehensive evaluations using appropriate tools and procedures.
- Review patient records for current diagnosis, past medical history, precautions, and contraindications.
- Establish measurable goals and develop a treatment plan to enhance patient independence/function.
- Re-assess treatment plan effectiveness and patient progress regularly.
- Communicate patient needs and progress to the treatment team, physician, and family members.
- Consult with medical providers regarding patient evaluation and treatment progress.
- Recommend and assist in securing adaptive equipment for patients.
- Maintain and submit required documentation in line with regulations and standards.
- Supervise Physical Therapist Assistants and Aides.
- Attend and contribute to internal staff development programs and maintain clinical practice skills.
- Valid Physical Therapy license
- Proficiency in Oasis and Home Health software (HCHB must-have)
- Experience in home health therapy is a plus
