Join Our Team as a Nurse Practitioner

We invite you to be part of our mission to deliver primary medical management in partnership with our respected PACE Organization (PO) Physicians. In this full-time role as a Nurse Practitioner, you will perform assessments, create care plans, and ensure that participants receive thorough care in a nurturing and supportive atmosphere.

Your Responsibilities

• Work collaboratively with PO physicians to conduct assessments and implement care plans.
• Engage in discharge planning and provide treatment for acute illnesses.
• Conduct home visits as required.
• Assist in developing clinical policies and procedures.
• Participate in the provider on-call rotation.

Qualifications and Requirements

• Master's degree in Nursing.
• 5-7 years of experience, ideally in long-term care settings.
• Active nursing license in North Carolina.
• CDS Certification, DEA Registration, and Board Certification are mandatory.
• Current BLS certification.
• Willingness to travel within the geographic area of the Center.
