This role requires a commitment to providing high‑quality patient care in a collaborative environment. Responsibilities Provide patient care in accordance with established protocols and standards. Collaborate with interdisciplinary teams to develop and implement care plans. Monitor patient progress and adjust care plans as necessary. Document patient interactions and interventions accurately in the EMR system. Required Experience / Certifications / Licensure Minimum 2 years of experience in an LTACH or similar rehabilitation environment. Valid state licensure required.
