Role Responsibilities
- Deliver thorough, ongoing medical care to individuals and families, spanning from childhood through end-of-life care.
- Oversee long-term health conditions, preventive care, acute ailments, and mental health issues with a patient-centered mindset.
- Perform physical examinations, analyze diagnostic results, and craft personalized health plans.
- Engage with care coordinators, specialists, and behavioral health experts to ensure comprehensive care delivery.
- Contribute to quality enhancement initiatives that improve patient outcomes and operational efficiency.
- Utilize electronic health records to ensure patient documentation is precise and current.
- Provide care through in-person consultations and virtual visits to meet varied patient preferences.
- MD or DO degree from a recognized medical institution.
- Completion of a Family Medicine residency program accredited by the ACGME.
- Board Certification in Family Medicine.
- Possession of an active Texas medical license or eligibility to obtain one.
- BLS and ACLS certifications preferred.
- Strong clinical acumen with a focus on preventive and proactive health measures.
- Familiarity with value-based care frameworks and a commitment to ongoing improvement.
