Responsibilities Assist in the facilitation of eligibility reviews for case management and waiver services. Support case management and waiver service coordination functions as needed. Conduct physical, functional, social, environmental, financial, and mental health assessments using clinical interview skills. Prioritize tasks effectively to meet deadlines and service requirements. Maintain clear, persuasive, and professional verbal and written communication with clients, families, and team members. Utilize computer systems proficiently, including Microsoft Suite applications. Adhere to universal precautions and infection control protocols. Apply basic math skills as required for documentation and reporting. Maintain awareness of community resources and services to support client needs. Qualifications Associate Degree is desired; additional equivalent experience may be substituted for part of the education requirement. Excellent verbal and written communication skills with persuasive ability. Customer-focused care approach in all communications. Ability to prioritize multiple tasks effectively. Possess clinical knowledge and interview skills necessary for comprehensive assessments. Computer literacy, including proficiency with Microsoft Suite.
