- Conduct evaluations for patients in both inpatient and outpatient settings and create tailored treatment plans
- Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
- Work closely with a team that includes nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
- Guide patients and their families on therapeutic goals, discharge strategies, and at-home exercise regimens
- Maintain thorough documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
- Engage in care team meetings and interdisciplinary discussions to ensure seamless patient care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- Current state licensure as a Physical Therapist
- CPR certification preferred
- Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
- Previous experience is highly desirable
- Ability to care for patients with intricate medical and mobility challenges
- Excellent communication and collaboration skills
- Comfort with digital charting and clinical documentation software
