Position Overview

The Canyons Post-Acute is seeking a compassionate and dependable Registered Nurse (RN) to join our nursing team. The RN will provide quality nursing care, monitor residents' health, coordinate care plans, and support the nursing team in maintaining resident safety and well-being.

Responsibilities

• Assess residents' health and monitor changes in their condition.
• Develop and update nursing care plans.
• Provide nursing care and oversee resident care delivery.
• Administer medications and treatments according to physician orders and facility policies.
• Report significant changes in residents' conditions to physicians and appropriate healthcare providers.
• Delegate and supervise appropriate duties performed by LVNs and CNAs.
• Communicate with residents, families, physicians, and the healthcare team.
• Maintain accurate and timely clinical documentation.
• Follow infection control, medication safety, and emergency procedures.
• Protect residents' dignity, privacy, safety, and well-being.

Qualifications

• Active California RN license.
• Graduation from an accredited nursing program.
• Current CPR/BLS certification.
• Strong clinical assessment, communication, and documentation skills.
• Ability to lead, delegate, and work well with a healthcare team.
• Previous skilled nursing facility (SNF) or long-term care experience preferred.
