Job Title: Hospice RN - Home Health Profession: RN (Registered Nurse) Specialty: Hospice Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8 hours (8:00 AM - 5:00 PM) Experience: Minimum 1 year in hospice care and home health License: Valid Driver's License & Insurance Certifications: BLS Certification Must-Have: Home care experience and background in home health Hospice experience Description: This position involves providing hospice care to patients in their homes. Clinicians will be expected to call patients to schedule visit times. On-call duties include one night per week and every third weekend. Coverage area includes specific counties. No clinical involvement; profiles go directly to the manager for consideration. Clinical duties include Start of Care, recertifications, and daily visits. Experience with hospice and home care is required. Patient demographics include pediatrics and adults. Home health aides may be present to assist with care. There is no point system in place for patient visits. Scrubs can be of any color. The schedule does not require requirements on holidays.
