Responsibilities Evaluate and treat patients across a wide range of diagnoses. Maintain accurate and timely documentation in accordance with facility guidelines and procedures. Qualifications Current and active state Physical Therapist license — or eligibility for licensure in the state of assignment. Graduate of an accredited Physical Therapy program.
