Licensed Practical Nurse (LPN)

Job Summary:

We are seeking a compassionate and dependable Licensed Practical Nurse (LPNs) to join our team at Washington Rehabilitation and Nursing Center in Chipley, Florida. As an LPN, you will play a vital role in providing high-quality care for our residents while maintaining a safe and respectful environment.

Responsibilities:

• Deliver direct nursing care to residents including medication administration and treatments as ordered by the physician

• Identify and report all abnormal changes in resident’s condition to RN Supervisor

and Physician.

• Provide wound care and other clinical interventions.

• Ensure accurate and timely documentation is informative and in a descriptive manner.

• Admissions, Discharge and Transfer of Residents.

• Supervise CNAs to ensure adherence to care standards in accordance with current rules, regulations, and guidelines that govern long term care centers.

• Maintain infection prevention and control practices.

• Support implementation and updates of individualized care plans for each resident.

• Safeguard resident rights, privacy, and dignity (HIPAA compliance).  

• Notify the residents' attending physicians and next-of-kin when there is a change in the residents' conditions or if the residents are involved in an incident or accident

• Attend and participate in all center in-service programs as required.

  Qualifications:

• MUST hold a current and valid LPN license in the state employed

• Long term care/post-acute care setting preferred

• Completion of postsecondary education

• Capable of maintaining regular attendance (i.e., weekend shifts and overtime) as required

Available Schedules:

• 7AM-7PM

• 7PM-7AM

License/Certification:

• BLS Certification (Preferred)

• LPN (Required)

Physical setting:

• Long term care

• Rehabilitation center

Weekly schedule:

• 3x12 alternating

• 4X12 alternating
