Community Health Navigator & Advocate

BEHAVIORAL HEALTH SERVICES INC in Gardena, CA is seeking a Community Health Worker to serve as a liaison among patients, families, healthcare providers, and community resources within an FQHC and behavioral health setting. The CHW will support patient engagement, health education, care coordination, outreach, navigation, and social support services to improve health outcomes and reduce barriers to care.
