The Community Health Center of Buffalo, a Federally Qualified Health Center (FQHC), is actively seeking an experienced Family Planning Nurse Practitioner to provide comprehensive outpatient care to patients at our modern facility. Minimum Qualifications:

• Licensed NYS, board-certified, Registered Professional Nurse Practitioner.
• Experience as a Family Planning Provider, including Long-Acting Reversible Contraceptives.
• Computer literacy with proficiency in MS Word, Excel, and Outlook. Experience with eClinical Works is preferred.

Qualifications:

• Active Nurse Practitioner license.
• Certification in Family/Adult Health Nurse Practitioner.
• In-depth knowledge of family planning and contraceptive methods.
• Excellent communication and counseling skills.
• Strong commitment to providing compassionate and nonjudgmental care.

Responsibilities:

• Conduct comprehensive health assessments for individuals seeking family planning services.
• Develop personalized family planning strategies based on individual patient needs.
• Educate patients on various contraceptive options, including benefits and potential side effects.
• Prescribe medications and contraceptives as needed.
• Collaborate with interdisciplinary healthcare team members to ensure holistic patient care.
• Maintain accurate and up-to-date medical records.
• Adhere to all relevant laws and regulations governing family planning services.
