Job Description

Position Title: Case Manager RN

Location: Wakulla Springs, FL

Shifts: Day shift with 1 weekend per 4 week schedule.

Job Summary:

The RN CM Care Coordinator will identify potential barriers to patient throughput and quality outcomes and will facilitate appropriate discharge plans.

Qualifications:

3 years of recent acute care Case Manager experience or 3 years in Med/Surg, Tele, Neuro, ICU, PCU, or ED
Associates Degree in Nursing or Diploma in Nursing required
Current FL RN license required or appropriate compact licensure.
