Hospice Certified Nurse Assistant

University Home Care Inc is seeking a compassionate and dedicated Hospice Certified Nurse Assistant to join our team in Livonia, MI. This role involves providing high-quality care and support to hospice patients, ensuring their comfort and dignity during end-of-life care. The ideal candidate will have a strong commitment to patient-centered care and the ability to work collaboratively within a multidisciplinary team. Responsibilities

• Assist hospice patients with daily living activities including bathing, dressing, grooming, and mobility.
• Monitor and report changes in patients’ conditions to nursing staff and healthcare providers.
• Provide emotional support to patients and their families during end-of-life care.
• Maintain accurate and timely documentation of care provided.
• Ensure compliance with hospice care standards and protocols.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.

Requirements

• Current certification as a Nurse Assistant (CNA) with hospice experience preferred.
• Compassionate and patient-focused attitude.
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and as part of a team.
• Basic computer skills for documentation purposes.
• Flexible availability to meet patient needs, including potential weekends or evenings.
