Admission Licensed Practical Nurse

$1,500 Sign-on-Bonus! M-F 8am – 4:30pm The LPN Admissions Care Coordinator primary role will be responsible for reviewing newly admitted hospice patient’s charts for accuracy, QA all necessary forms needed for admissions, and follow up with patients on the NTUC list to provide additional information about hospice services. Responsible for answering incoming telephone calls from patients, caregivers, and medical professionals. Ability to react quickly and calmly, exercise good judgement, and make sound decisions during both urgent and routine communications. Proficient in receiving, obtaining, and relaying accurate information to be disseminated. Provides skilled, supportive patient and family care over the telephone in accordance with certification restrictions. May be required to visit patients when needed. Provides care within scope of practice. Qualifications Current Florida LPN license required. Current CPR certification. Minimum of three years of nursing experience in a home health/hospice setting within the past five years. Preferred knowledge of hospice, Medicare, Medicaid, and private insurance benefits. Ability to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality. Self‑directed and able to work independently or as a team. Basic computer skills, use of common office software, and EMR software experience. Excellent people and relationship‑building skills. Excellent customer service skills. Maintain effective work relationships with co‑workers, the public, and administration. Highly organized, adaptable problem solver, ability to work under stress or pressure. Uphold Treasure Coast Hospice/Model Standards for patient visits and compliant documentation. Understand and execute routine Hospice Nursing: crisis intervention, communication skills, teaching skills, thanatology skills, and needs. Under the supervision of the RN Admissions, provide skilled professional nursing care to the patient and family unit incorporating physical, psychosocial, spiritual, cultural, and biological components. Utilize the nursing process, demonstrate accurate data collection, and assist in the planning, implementation, and delivery of clinical care services. Implement the nursing plan of care in accordance with physician orders and under the RN Case Manager. Provide appropriate pain and symptom management, including patient’s response to treatment and medications. Document patient care reflecting nursing intervention, patient’s response to care, and progress toward goals as set in the Plan of Care. Maintain clerical type records via computer or handwritten data. Provide an effective, supportive, sensitive approach in response to needs of patients, families, and staff; remain polite and courteous at all times. Provide program development strategies for Treasure Coast Hospice in an organized, routine capacity that enables the cohesion of team developments. Teach and uphold Treasure Coast Hospice’s standards with a focus on perfect documentation, visits, communication, and professional dress & behavior. Lead, develop and motivate employees with confidence, energy, integrity, and resources. Provide verbal assistance over the telephone and care to patients and families. Participate as an active supporter of the Treasure Coast Hospice compliance program, report as required, and comply with all Federal, State, and Local laws and the organization’s compliance program. Fulfill all responsibilities related to the success of the organization’s strategic plan, as requested and assigned.
