RN Case Manager - Care Coordination & Discharge

Oswego Health is seeking a Case Manager RN to collaborate with multi-disciplinary teams ensuring high-quality patient care. This role involves coordinating individual patient management and performing assessments and quality reviews. The ideal candidate must be a graduate of an approved RN program, hold current NYS licensure, and have a minimum of 5 years clinical experience.
