RN Diabetes LEAP Nurse - Clinic - Coweta

Job Title

The purpose of this position is to organize, coordinate, implement, evaluate and provide lower extremity preventive services throughout the MCNDH clinics.

Minimum Qualifications

Education Preference: Graduation as a BSN RN curriculum from an accredited professional school of Nursing

Experience Three (3) years relevant experience is preferred.

Licenses & Certification Must possess current State of Oklahoma Nursing License. Certification in Diabetes Foot Care or/and Wound Care is preferred.

Knowledge & Skills

• High degree of knowledge and skills in providing state-of-the-art foot care.
• High degree of knowledge in health care delivery and quality improvement.
• High degree of knowledge and skills in providing preventive and treatment interventions to persons with diabetes in outpatient settings including initial, routine, urgent, annual and complication care.
• Skill in dealing analytically and systematically with problems of organization, workflow, data systems and planning.
• Knowledge of ADA, IHS, MCNDH and Diabetes Program guidelines, regulations, policies and procedures.
• Knowledge and skills to provide preventive and treatment interventions to persons with diabetes in an outpatient setting including initial, routine, urgent, annual and complication care.
• Ability to provide diabetes education effectively to groups and individuals.
• Skill in obtaining departmental information from patients.
• Ability to communicate courteously and effectively with patients and their families, medical and administrative staff, health care professionals and the general public both orally and in writing.
• Ability to utilize a personal computer in a Windows environment.
• Ability to maintain a confident professional demeanor.

Job Summary

The purpose of this position is to organize, coordinate, implement, evaluate and provide lower extremity preventive services throughout the MCNDH clinics. The incumbent will work with staffs of the Diabetes Prevention and Management Program and other MCNDH clinic staff to accomplish these services.

Work Environment

Work is performed in an office and clinical environment with normal risk of exposure to the hazards associated with each setting. There is regular and recurring risk of exposure to infectious communicable diseases, potentially hazardous chemicals and bio-hazardous materials. Incumbent must be aware of all risks involved and be willing to take necessary precautions. Some travel may be required.

Physical Demands

The work requires normal demands of working within the MCNDH clinics and offices. Driving a GSA vehicle may be required to travel to other MCNDH facilities. Must have valid license and be insurable.

Essential Functions

Satisfactory job performance will be determined by successful execution of the following:

1. Assesses current foot care needs and services provided through the MCN diabetes population for all three levels of prevention, reports findings and makes recommendations to the MCN Diabetes Program Manager.
2. Works with assigned clinic staff and diabetes program staff to develop, implement, evaluate and sustain diabetes foot care using the IHS and ADA Best Practices guidelines.
3. Works with diabetes staff in establishing, monitoring and evaluating diabetes foot care tracking systems that assist with provision of timely care and program improvement.
4. Establishes routine opportunities in which to communicate with primary care providers, diabetes educators, dietitians and clinic nurses regarding individual and general foot care plans.
5. Maintains available or attainable resources as indicated by program evaluation or daily observations.
6. Participates in establishing proficiency monitors for care provided at each level of foot risk.
7. Facilitates and supports foot care delivery that is patient-centered and sensitive to clinic flow issues.
8. Organizes workflow, data management and planning activities within available time frames.
9. Ability to communicate courteously and effectively with patients and their families, medical and administrative staff, health care professionals and the general public both orally and in writing.
10. Ability to utilize a personal computer in a Windows environment.
11. Ability to maintain a confident professional demeanor.
12. Participates with the Diabetes Self-Management Team to develop or adapt curricula for diabetes foot care for diabetes self management education at all levels of foot risk and for brief education encounters by the primary care provider staff.
13. Maintains professional licensure/certifications.
14. Regular attendance is required.
15. Performs other duties as required.
